We have two locations serving Northwest Arkansas in Fayetteville and Springdale.  If you are a new patient and would like to set up an appointment, please feel free to call our referral coordinator at (479)-751-3900. If you need to send us anything via mail, our mailing address is as follows: PO Box 871, Tontitown, Arkansas 72770.

Please take note that we are closed on weekends. An asterisk indicates that on that day of the week, the last appointment is scheduled at 5:00 P.M. For your convenience, we have a bilingual therapist and receptionist at our Springdale Location. Our normal business hours are as follows:
